AFIUK is proud to be a delivery partner for the SNEE ICB Core20PLUS5 programme, working to tackle health inequalities in underserved communities. As part of this initiative, we have trained Community Connectors who play a key role in bridging the gap between local communities and healthcare services.

Our Connectors are trusted members of their communities who actively engage with residents, helping them access the support they need. With their deep understanding of local challenges, they provide valuable feedback to NHS services on the barriers people face and suggest practical solutions to improve access and quality of care.

Through this work, AFIUK is committed to ensuring that everyone, regardless of background, has the opportunity to live a healthier life.
